Puppy Adoption Form



We require all potential puppy buyers to fill out our puppy adoption form.
This gives us a better idea of your lifestyle, family, living situation etc
so we can provide you with the best puppy for your family.


We take placing our puppies VERY seriously. All puppies are sold on contracts tailored to each puppies specific needs. Please contact me for more info on my contract terms.

Our Puppy Placement Requirements:


We have raised all of our puppies
with alot of love and attention and hugs and kisses!
So our major requirement is
that you provide a safe, loving forever home
and that you will give our babies lots of love and attention!


1.Children: Unlike some other breeder,
who do not sell to families with young children,
we feel that Shih Tzu's are So Sweet and make
such Wonderful pets for all ages,
we are more than happy to sell to families with children
Our Shih Tzu's are raised in our home
around small children from the day they are born
So your new little puppy will have a head start!
We fell that growing up with animals
teaches children responsibilty and compassion for all animals.
We only ask (for the safety of the puppy)
is that if you do have young children
that you agree to supervise them at all times
and teach them to be gentle and loving when playing with the puppy.



2.Home Alone: Shih Tzu's are very social dogs
and love lots of attention and love
This is one of the reason they make such loving family pets
However it is not fair to them to be left alone all day
and kept in a crate with no company
There is a way to solve this if you happen to work very long hours
and that is, to get another dog or cat to be there
playmate and keep them company.
we do offer a discount if purchasing two babies. Ask us for details.*
But Shih Tzu's do best when they have someone around
most of the time to keep them company.
So PLEASE think long and hard and do your research
as a puppy is a LIFETIME commitment
and don't buy a puppy if you are not home enough to give
them all the love and attention they deserve!



3.Outside: Shih Tzu's when outside tend to wander like most dogs
They are just curious and there are so many new things to explore
Before you know it they can find themself lost and confused
So for your dogs safety, it is important
to have a securely fenced yard,
or some other safe place they can not wander from
So Please keep them safe at all times.

20. All of my puppies are sold on contracts tailored to each pets needs. One of the specification in my contract is that if at any time during the pets life you are no longer able or willing to keep him/her then you are to contact me and the pet is to come back to me; this is not negotiable. This Shih Tzu is to never be given away, sold, placed in rescue, animal shelter or a pet store. This includes giving this Shih Tzu to a friend, family member, neighbor etc. Do you understand this?
